Cardiomegaly, the abnormal enlargement of the heart, is a clinically significant radiographic finding routinely assessed on chest X-rays. The current diagnostic reference, the cardiothoracic ratio (CTR), reduces the rich two-dimensional information in a heart-and-lung segmentation mask to a single linear measurement, discarding shape-related information such as curvature, symmetry, and orientation. This report reviews the state of the art in automated cardiomegaly assessment and identifies a persistent gap: even the most advanced multimodal approaches describe the cardiac silhouette using only two scalar biomarkers, the CTR and the cardiopulmonary area ratio (CPAR), despite the richer geometric information already contained in the segmentation masks from which they are derived. Building on this review, the report proposes extracting an extended set of interpretable geometric descriptors from these masks and integrating them into a multimodal architecture combining image features, clinical data, and digital biomarkers, following the fusion framework of Duvieusart et al. (2022).*** La cardiomégalie, l’élargissement anormal du coeur, est un signe radiographique cliniquement significatif, systématiquement évalué sur les radiographies thoraciques.
